Step 1. Forms
Returning patients: If you have visited us before, you will simply be asked to review these forms to be sure your information is up to date.
New patients: Upon your arrival, our front desk staff with ask you to fill out the following forms:
- Patient Demographics
- Medical History
- Privacy Consent
This form is for your basic information (name, birthdate, etc.), contact information, primary doctor,
and insurance.
You will list here your medications, allergies, and medical/surgical history. The second page is a systems review, to be
completed by you and reviewed by the physician on the day of your visit.
This document describes how medical information about you may be used and disclosed and how you can get access to this information.
Please sign and return the last page.
Please click on the links to download and print the forms. Filling them out ahead of time will reduce your wait and save you time on the day of your appointment.
Step 2. Physician consultation
After completing the necessary paperwork, a nurse or medical assistant will bring you to a clinic room, where you will wait to see one of our physicians. Depending on the nature of your visit, the physician may recommend one of a variety of items after your consultation. This may include additional tests, a vascular lab, an office procedure, or a hospital procedure. Some of these items can be completed on the same day as your visit.
The nurse or medical assistant will often stay in the exam room with the physician during your consultation. They will give you the proper forms, along with instructions on how to proceed with the doctor's orders.
Step 3. After your consultation
At the end of your visit, you will return back to the front desk area, where our experienced front desk staff will work with you to schedule any tests, surgeries, or follow-up vists that may have been ordered. You will receive an appointment card with the relavent dates on it.
